1. Following are essential documents, to constitute a Death and obtain Death Certificate:
a) Medical Certificate, issued by a Registered Medical Practitioner, certifying the Cause of Death, Date & Time & Place of Death and Complete Name, Age, Address of deceased.
b) Recently, Aadhar Card number is made mandatory, for Death Certificate, by Govt.
c) Receipt /Certificate, issued by local Cemetery /Funeral office
d) Death Certificate (Digital, from year 2017 onwards) can be download from Govt. online website: http://crsorgi.gov.in/

You need the receipt you got from the crematorium. You need to fill the form at the municipal Corporation with sub registrar oof birth and death. Once you submit the same they issue you death certificate.
